Your First Visit
At the consultation appointment, you will meet with Dr. Alfi. We will discuss your goals for treatment and outline all of your options. This appointment is a great time to ask questions and to learn about the many treatments available. We encourage you to write out a list of questions beforehand so that we can address all of your concerns.
What to Bring
In order to speed up your appointment time, you can submit new patient forms online. If you do not submit patient forms online, we ask that you arrive at your appointment a few minutes early to complete the necessary paperwork. Please bring these items with you to the consultation:
- Referral slip, if applicable
- X-rays if you have them; you can also request that your dentist forward X-rays to our office in advance
- Dental or medical insurance
- List of current medications
- Valid form of ID, such as a driver’s license
Parents of Minors
A parent or legal guardian must be present at the consultation appointment for patients under the age of 18.
What to Expect
You will receive an oral health examination at the first visit. This often includes 3D scans of the head, neck, mouth, and face to assess the condition or detect any other issues. Dr. Alfi will discuss in detail your options for treatment and anesthesia options if required for surgery. If surgery is necessary, we will schedule future appointments. We will also provide you with a cost estimate for treatment at this time.
Financial & Insurance Information
Alfi Oral, Dental Implant & Facial Surgery is in-network with many dental and medical insurance providers. To find out if we are in-network with your insurance provider, contact our office. We ask that you contact your insurance provider directly prior to your appointment for information about your coverage. It is your responsibility to know your coverage details and to notify us of any changes to your policy. If we are unable to verify your insurance eligibility, you are responsible for covering the cost of your visit at that time.
